I hereby authorize the staff of Xtreme Hoops to act for me according to their best judgement in any emergency requiring medical attention, and I hereby waive and release the facility and Xtreme Hoops from any and all liability for any injuries incurred while at any Xtreme Hoops facility. I have no knowledge of any physical impairment that would be affected by my participation in the Xtreme Hoops program. I also understand that Xtreme Hoops retains the right to use for publicity and/or advertising purposes any photographs of any participants taken at Xtreme Hoops events.
I understand and agree to pay a $150 per game
forfeit fee
should my team forfeit, plus my team will not be allowed to participate in an Xtreme Hoops Tournament for up to six (6) months following the forfeit.
